The Verification of Enrollment form is used to request an official letter from the Office of the Registrar stating that you are enrolled in courses at Cal Poly Humboldt. Verification of Enrollment letter contains number of units, academic standing, and start/end date of the semester.

Return your completed form to the Office of the Registrar:

  • In person, Monday-Friday, 8 a.m.-5p.m., SBS 133;
