Routing Instructions: 

Fee waiver applications use an online submission process which gives the opportunity to sign, gain signatures and submit electronically. The form will then be routed to the various departments involved through an electronic database. This form should be filled out each semester by any eligible staff member whose dependent wishes to take course(s) through the fee waiver program. Applications are due three weeks before the first day of instruction. Reminder: Fee Waiver cannot be used for Humboldt Extended Education classes including summer session.
