Use this form to request a change in type of culminating experience and associated coursework after advancement to candidacy.
Step 1. If you are changing to a thesis or project, open the Attachment B: Project/Thesis Description (Google Drive). You will be directed to make a copy. Complete the information and save.
If you are changing to a comprehensive exam, skip to step 2.
Step 2. If you are changing any of the courses for your progam, Open Attachment C: Course Changes. You will be directed to make a copy. Enter the courses you are removing and adding to your program. Save.
Step 3. After you have your attachments ready, open the Addendum to Advancement to Candidacy form. You will be redirected to Adobe Sign.
