Graduate: Addendum to Advancement to Candidacy
Use this form to request a change in type of culminating experience and associated coursework after advancement to candidacy.
Step 1. If you are changing to a thesis or project, open the Attachment B: Project/Thesis Description [2] (Google Drive). You will be directed to make a copy. Complete the information and save.
If you are changing to a comprehensive exam, skip to step 2.
Step 2. If you are changing any of the courses for your progam, Open Attachment C: Course Changes [3]. You will be directed to make a copy. Enter the courses you are removing and adding to your program. Save.
Step 3. After you have your attachments ready, open the Addendum to Advancement to Candidacy [1] form. You will be redirected to Adobe Sign.
