Students in F-1 or J-1 status should visit the Center for International Programs (CIP) to obtain a travel endorsement on Form I-20 or Form DS-2019 approximately two weeks before any planned travel abroad.

If you plan to attend another school in the U.S. you must request that your SEVIS records be released in order to receive your new I-20 or DS-2019 from your next U.S. school. Please submit this form to the Center for International Programs (CIP) along with a copy of your acceptance to the other university so we can release your SEVIS records.

Volunteers provide various types of services to the Sponsored Programs Foundation. This form Identifies the person as a Volunteer and makes them eligible for worker’s compensation insurance coverage if they are injured as a result of such volunteer services.

This form can be completed in place of submitting an original receipt for reimbursement. It certifies that you incurred the listed amount in authorized expenses for Sponsored Programs Foundation and the original receipts were lost, as well as that these expenses have not been reimbursed by any other party.
