If your department is employing a student who will be driving a state vehicle for university business, please fill out this form and send it to Facilities Management in order for us to maintain our vehicle records. Please ensure that your student meets all of the requirements of the CSU guidelines for driving a vehicle for university business.
The California State University offers eligible employees the opportunity to enroll in the CSU Employee Fee Waiver and Reduction Program. This program provides for the waiver or reduction of certain fees for employees who enroll in work‐related courses offered by the CSU for the purpose of improving skills for existing jobs, or advancement in accordance with a career development plan.
The purpose of this form is to document that each new employee (both citizen and noncitizen) hired after November 6, 1986, is authorized to work in the United States.
This form is required from the State and is used prior to employment or to make changes during employment, for determining exemption status for taxable deductions, and any other deductions sought.
