This form is used to create a new course service learning course or convert an existing course to include a service learning component.
Routing Instructions:
Complete both the Service Learning "S" Course Designation form and a New Course Proposal. Original forms plus a syllabus are submitted to the Integrated Curriculum Committee (ICC) and copies of all 3 documents to the Center for Service Learning & Academic Internships.
