INSTRUCTIONS:
- Form must be completed by the vehicle driver involved in the accident within 24 hours of the accident
- Supervisor must sign at the bottom as "Reviewing Officer" and must complete the STD 274 form http://www.documents.dgs.ca.gov/dgs/fmc/pdf/std274.pdf
- Supervisor will then submit both the STD 270 and STD 274 form to the Risk Management Office in SBS 345 within 48 hours of the accident
