This form is used to document approval of a student's Individualized Degree Plan (IDP) of Study Proposal.
Routing Instructions:
The Individualized Degree Plan of Study Approval form must be initiated by the IDP Director. Forms initiated by students will not be processed. This form uses Adobe Sign for approval and routing.
