Use this form to initially establish a group of Humboldt Users whose Humboldt Username Passwords need to be held to an aging standard due to audit requirements, state or federal law, or Humboldt Departmental Policy. See following Humboldt IT Procedure HSU-004 for more information.

All full-time faculty unit members must provide a written statement to the appropriate administrator for all Outside Employment, when such employment is expected to amount to more than 160 hours per semester for academic year or 10-month faculty unit members, or 120 hours per 3 month period for faculty unit members holding 12-month appointments.
