Please ensure that the beginning and ending dates of the month in which the appointment will occur are entered on the form and that a new form is submitted each pay period the substitute appointment is needed.
JOB DESCRIPTION: The Resident Mentor (RM) is responsible for a group of approximately ten high school students during our academically rigorous 5 week long Summer Academy at Cal Poly Humboldt. Duties will also include being a tutor, advisor, disciplinarian, role model, mentor, and leader of the core group.
At the end of every semester, or at the end of the work assignment, use this form to provide each student employee with an evaluation of his or her performance.
