Must be approved by College Dean/University Library Dean/Director of CAPS/Director of Athletics and reviewed by Academic Personnel Services and the VP for Student Affairs(for Counselor positions only) or the President (for Coach positions only) prior to selection of final candidate.

An FCOI (Federal Conflict of Interest) form is required by some funding agencies in addition to the 700-U. Like a 700-U Form, the FCOI is required at the start and close of each project but must also be renewed annually.
