The CSU Family Medical Leave (FML) provides employees with unpaid leave time for up to twelve (12) weeks to care for self or eligible family members. The CSU FML incorporates both the Federal Family Medical Leave Act (FMLA) and California Family Rights Act (CFRA) leave entitlements and run concurrently.

Use this form to request a specific title for an independent academic work (IAW) course. Specific course titles may only be submitted to the Office of the Registrar for inclusion in official records where external requirements such as accreditation, professional standards or certification require the use of a specific course name.

Most students receive F-1 visas from their home country and enter the U.S. in F-1 status if they will pursue a full course of study in an academic institution or engage in language training. Some students are married to another F-1 visa holder and wish to change their status to a dependent status of F-2.

In order to obtain F-1 status, you must have been admitted to a full-time course of study at Humboldt State University and have sufficient financial resources to meet your program's tuition, fees, and living expenses. You must receive a “change of status” form I-20 from the Center for International Programs (CIP) for F1 study before you can apply for a change of status.
