If your culminating experience is a comprehensive exam go to step 3.
This form is used to request authorization to enroll in one unit of a discipline-specific x693 course through the College of Extended Education & Global Engagement. Note: x693 units are not eligible for financial aid.
A written assessment for Academic Student Employee (ASE) performance. A blank copy of this evaluation form must be given to the ASE within the first fourteen (14) days of appointment if an evaluation is to take place.
A general description of duties for the position that specifies the faculty member or supervisor to whom the individual will report, the location where the work will be performed, the class assigned, if applicable, the time and place of any applicable orientations, and duties and expectations of the appointment. This form is required for all Unit 11 employees per the CBA.
Cal Poly Humboldt requires that each college within the University develop a field trip policy appropriate to its field trip activities. A committee composed of members of the college and the university's risk manager will work in conjunction to develop these policies. All policies will receive final approval from the dean of the college and the vice president.
