This form should be used by students who advanced to candidacy prior to Spring 2013, when the new Application for Advancement to Candidacy and Graduation form was used.
Use this form to request changes to your advisor, committee member or coursework, after you have advanced to candidacy. Complete one form for each type of change.

Completed forms are sent via certified mail to the student grievant and respondent(s)if living off campus. If parties are on campus, the form can be hand-delivered and signed for, or the grievant / respondent can pick up the form in the dean's office and sign for it. Completed forms are also sent to the Grievance Coordinator via campus mail.
Use this form to petition to apply credit for a course taken during the final semester of your senior year towards a graduate program.
Step 1. Open the Candidacy Attachment A: Proposed Graduate Course List (Google Drive). You will be directed to make a copy. Fill in the list of courses for your program and save. You will upload this attachment in Adobe Sign.
If your culminating experience is a comprehensive exam go to step 3.
