The Grade Appeal Policy establishes a process for students to redress their concerns about certain academic decisions made by faculty. Read the policy to become familiar with the circumstances under which to use this webform.
As an undergraduate student, you may petition to have the first "WU" grade(s) earned at Cal Poly Humboldt excluded from your GPA. The “WU” grade will remain on your transcript, but will not be included in GPA calculation.
Make changes to your graduate course list that are associated with a change of culminating experience after advancement to candidacy.
If your culminating experience is a project or thesis, open Attachment B: Project/Thesis Description (Google Drive). You will be directed to save a copy. Enter the requested information and save. You will upload a copy to your Application for Advancement to Candidacy and Graduation in Adobe Sign.
Open the Proposed Graduate Course List (Google Drive). You will be directed to save a copy. Save the fillable form in case you need to make corrections. Fill in the list of courses for your program and save. You will upload this attachment to your Application for Advancement to Candidacy and Graduation in Adobe Sign.
If you have a current Incomplete Grade Contract (no final grade assigned), you are eligible to request an extension of up to one additional year with this petition.
Use this form if you already hold a bachelor's degree (post-baccalaureate student) and are working toward a second bachelor's degree. If you do not currently hold a bachelor's degree, apply for graduation through your Student Center.
