Request to offer appointment to final candidate for Probationary Faculty Unit Employee recruitment. To be completed by the Office of the Dean/Director in consultation with the Provost and Vice President for Academic Affairs/Vice President for Student Affairs/President prior to verbal offer.
To be submitted annually by current coaches, counselors and librarians to request a subsequent appointment.
To be used by new applicants for temporary Coach, Counselor or Librarian appointments. Application will be kept on file for 3 years.
Use for temporary Librarian new appointments, revisions, or reappointments.
Use for tenure-track faculty wishing to distribute workload over three terms
Recommendation report to Dean/Director of final candidates for Probationary Faculty Unit Employee recruitment. Must be approved by College Dean/University Library Dean/Director of CAPS/Director of Athletics and reviewed by Academic Personnel Services and the Provost and Vice President for Academic Affairs/Vice President for Student Affairs/President prior to selection of final candidate.
